בפניות מסוג אלו יש למלא את השדות הבאים:
- מוען
- נמען
- תאריך ושעה מדוייקים
- במידה ויש הודעת שגיאה שקיבלת נא לצרף אותה
בכדי לייעל את הטיפול בתקלה, יש למלא את כל השדות.
